Tour Forms & Registration
Here’s the Drill
To register for a Vietnam Battlefield Tour, please download, complete, and submit the following required forms:

What to Submit With Your Forms
Along with your completed forms, please include the following items:

A clear, full-color copy of your Passport Information Page with the adjoining signed Signature Page
-
Important: Do not cut off any numbers at the bottom or sides of the passport pages
-
A sample of the required passport copy is included in the Vietnam Visa Application package

Two (2) clear, full-color 2×2 passport photos
-
A $500 deposit per person to secure your spot on the tour
Your deposit is refundable up to 120 days prior to departure if you need to cancel.
Special Interests & Tour Customization
We will incorporate your special interests into the tour itinerary as much as possible. The earlier we receive your information, the more individualized your tour experience can be.
Passport Requirements (Important)

Your passport must be valid for at least six (6) months beyond the end date of your tour

Your passport must have at least two (2) blank visa pages available
If you do not currently have a passport, start the process now. Your updated passport information can be added later.

These Terms & Conditions apply to all Vietnam Battlefield Tours.
Required Forms
VBT Registration Form (Required)
This form must be completed and submitted with your deposit to be officially registered for a tour.
VBT Release of Claims Form (Required)
This form must be signed and returned along with your Registration Form.
VBT Medical Disclosures Form (Required)
This form must be signed and returned along with your Registration Form.
Vietnam Visa Application (Required)
This package includes:

Instructions for submitting a color copy of your passport

Passport photo requirements
-
Step-by-step instructions for completing the Vietnam Visa Application
Important: Send this form only to Vietnam Battlefield Tours along with your other completed forms.
Do NOT send it to the address listed on the Visa Application.
Optional Forms
VBT Special Request Form (Optional)
If you have specific locations or interests you would like included during the tour, please complete this form.
VBT Alternative Gateways Form (Optional)
Starting January 2026, all tours will depart from George Bush Intercontinental Airport (IAH) – Houston, TX.
If you wish to depart from a different city than the one scheduled for your tour, please complete this form.
